This is a fully remote position, open to applicants in Connecticut, +10 more states.
• Provide patient care services utilizing a remote technology platform.
• Deliver comprehensive, evidence-based care autonomously, which includes assessment, diagnosis, treatment, health problem management, health counseling, and planning for patient disposition.
• Educate patients regarding pregnancy prevention, STI prevention, and safer sex practices.
• Assess and evaluate patients needing primary care, those who are acutely ill, chronically ill, and those in transitional care.
• Offer patient counseling, contraceptive care guidance, and manage medications.
• Instruct patients on health maintenance and address their care inquiries.
• Record patient care activities in an EHR system in adherence to MinuteClinic policies and procedures.
• Collaborate on patient care with internal and external colleagues, including those within the broader patient-centered medical home framework.
• Function within a patient care team comprising Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare professionals.
• Operate independently, prioritize tasks, resolve issues, take initiative, and advocate for both patients and the practice.
• A current, unrestricted license in one of the following states: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV.
• A minimum of two years of relevant medical experience or an equivalent background.
• Completion of a Master’s Degree in a Family Nurse Practitioner program.
• Valid National Board Certification.
• A state license to practice in the Advanced Practice Nurse role.
• Strong verbal, written, and electronic communication abilities.
• Excellent organizational skills with a proven ability to manage multiple tasks effectively.
• Demonstrated initiative, problem-solving capabilities, adaptability, and flexibility.
• Ability to work autonomously without direct supervision.
• Proficient in information management and technology systems.
• Capability to collaborate effectively with professional colleagues.
• Depending on the market, proficiency in additional languages, including Spanish, may be necessary.
• Access to broadband connectivity.
• A quiet environment with a neutral background for remote consultations.
• Commitment to uphold patient privacy in accordance with CVSH guidelines.
• Ability to view and read on a computer screen and other electronic devices.
• Willingness to work a full-time schedule on weekend evenings: three weekdays and every Saturday and Sunday from 2:30 p.m. to 11:00 p.m.
• If hired, readiness to obtain any additional licensure as required.
