This is a fully remote position, open to applicants in United States.
• Assist in coordinating safe, high-quality virtual care for low-acuity, non-emergent medical issues through video, phone, and asynchronous messaging.
• Perform patient intake and gather health requirements and objectives.
• Assess health conditions and care necessities.
• Review medical records, referral documents, and test results; integrate findings into care plans and communicate with relevant providers.
• Direct patients towards suitable therapies available through Ovatient and its partners.
• Record concerns, inconsistencies, and other issues discovered during evaluations.
• Encourage continuity of care while minimizing fragmentation, duplication, and treatment gaps.
• Conduct technology assessments and assist patients in navigating care technologies.
• Collaborate closely with Ovatient providers, the Behavioral Health Team, registered nurses, and interdisciplinary healthcare professionals.
• Oversee and manage referral queues, process behavioral health referrals, and facilitate connections to specialized services or community resources.
• Monitor patient communications, respond promptly, address concerns, offer guidance, and escalate issues as required.
• Guide patients through onboarding to treatment programs, self-guided resources, content libraries, and secure asynchronous messaging.
• Deliver empathetic, patient-centered care across a range of populations.
• Maintain precise and up-to-date electronic medical records while ensuring data security and confidentiality.
• Engage in continuing education, quality improvement initiatives, and clinical team meetings.
• Keep abreast of advancements in virtual care, clinical best practices, and virtual care technologies.
• Completion of an accredited medical assistant program, nursing assistant program, or possess an LPN with a compact license.
• Active national certification in medical assisting from a recognized organization such as AAMA or NHA.
• A minimum of 5 years of clinical experience in primary care or urgent care settings.
• Intermediate to advanced computer skills, including the ability to navigate multiple systems on a single monitor and quickly learn software programs and applications.
• Strong expertise in telemedicine and virtual care technologies.
• Experience and skill in using Electronic Medical Records (EMR) software.
• Exceptional communication, interpersonal, and problem-solving abilities.
• Proficient telephonic and virtual communication skills with a proven record of delivering excellent customer service.
• Strong dedication to patient care, quality, and clinical excellence.
• Ability to work independently in a fast-paced, dynamic environment.
• Availability to work between 7 am–4 pm EST.
