This is a fully remote position, open to applicants in United States.
• Evaluate patient needs through telephone consultations utilizing the nursing process and established guidelines.
• Identify the suitable level of care, and provide educational resources, care plans, interventions, and follow-up instructions to patients.
• Collaborate and escalate concerns to on-call providers when further guidance is required.
• Prioritize patient interactions based on acuity and necessity.
• Utilize evidence-based practices to ensure quality patient care.
• Implement measures to mitigate patient risk and enhance safety.
• Evaluate the readiness of patients and families to learn, tailoring educational approaches accordingly.
• Support initiatives in shared governance and process enhancements.
• Engage in professional development activities, mandatory meetings, educational programs, and annual competency assessments.
• Communicate and collaborate with clinicians and healthcare team members to facilitate patient-centered care.
• Identify and report patient safety incidents and near misses, assisting in corrective measures.
• Utilize ethical decision-making while functioning as an effective member of the patient care team.
• Schedule appointments in accordance with protocol recommendations.
• Coordinate medical and nursing management tasks, including procedures and medication refills.
• Maintain and update clinical records in compliance with agency, state, and federal regulations.
• Document call interactions in the Electronic Medical Record system.
• Relay patient status and care plan updates to physicians and interdisciplinary team members.
• Participate in team meetings and undertake special projects as assigned by leadership.
• Contribute to health information resources, system efficiencies, and operational enhancements.
• Assist in interpreting departmental policies and advising staff on procedural modifications.
• Current Registered Nurse license issued by the state where the team member practices.
• Associate's Degree (or equivalent knowledge) in Nursing.
• At least 2 years of acute care experience, preferably within the last 4 years; experience in ED, Urgent Care, ICU, and some Medical-Surgical units considered.
• 2+ years of experience in ED and/or Urgent Care is highly preferred.
• Strong critical thinking and problem-solving abilities.
• Knowledge of healthcare specialties, levels of care, symptom recognition, treatment recommendations, and established protocols.
• Exceptional verbal communication skills, empathy, active listening, diplomacy, and customer service proficiency.
• Ability to organize and prioritize tasks, manage multiple responsibilities, and maintain flexibility in scheduling.
• Proficient with computers, internet, desktop software applications, and multi-line telephone systems.
• Capability to research and document calls across multiple systems.
• A stable and secure internet connection is required.
• Must possess functional vision, touch, speech, and hearing capabilities.
• Must have a quiet environment for making and receiving telephone calls.
• Ability to lift 15 lbs.
• Availability to work every fourth weekend and participate in holiday rotations.
