Registered Nurse – Care Management, Evening & Overnight Schedule

This is a fully remote position, open to applicants in United States.

📋 Description

• Prepare detailed Annual Wellness Visit charts to enhance provider efficiency and ensure compliance with CMS regulations.

• Conduct face-to-face virtual consultations via Zoom to assess medical, surgical, and family histories; medications; screenings; immunizations; as well as cognitive, functional, and depression evaluations.

• Identify overdue screenings, risk factors, and gaps in care.

• Relay patient summaries and critical findings to healthcare providers.

• Manage complex medical cases through telephonic and portal-based care management.

• Execute thorough nursing assessments.

• Create, implement, and monitor personalized care plans for CCM, RPM, RTM, and APCM.

• Complete the development and annual updates of APCM care plans.

• Employ motivational interviewing techniques to engage patients and their caregivers.

• Track progress, reassess objectives, identify obstacles, and modify care plans accordingly.

• Update primary care providers and interdisciplinary teams on patient status and progress.

• Provide patient education, coaching, and advocacy services.

• Identify and address gaps in preventive care, laboratory tests, medications, and follow-up care.

• Coordinate orders, referrals, follow-up actions, services, and providers efficiently.

• Maintain accurate, timely, and confidential patient records.

• Utilize CT360, RXNT, Zoom, EHR systems, and telehealth platforms.

• Contribute to clinical quality, patient engagement, and satisfaction metrics.

• Comply with HIPAA, CMS, and organizational standards of compliance.

• Exhibit independent decision-making, prioritization, multitasking abilities, professional development, and perform other assigned tasks.

⛳️ Requirements

• Active and unrestricted Registered Nurse (RN) license in a compact state.

• Ability and willingness to secure additional state licenses upon hire.

• Minimum of 3 years of clinical nursing experience, including bedside, Primary Care, Med/Surge, or Home Health settings.

• Experience in providing care within a telehealth or virtual care environment.

• Previous remote work experience.

• Familiarity with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), telehealth or virtual care coordination, primary care, or care management.

• Proficiency in EHR systems and telehealth platforms.

• Strong clinical assessment and care coordination capabilities.

• Dedicated and private workspace at home.

• Operating system: Windows 10 or later, or macOS 14 Sonoma or later; Windows or Apple computers only.

• Processor: Intel Core i5 or AMD equivalent, or Apple Silicon.

• Minimum of 16 GB RAM.

• Wired headphones required.

• Internet speeds of at least 100 Mbps download and 50 Mbps upload, connected via ethernet.

• Use of Google Chrome with Amazon Workspaces.

• Webcam for video conferencing.

• Preferred qualifications include experience with Medicare Annual Wellness Visits, knowledge of CMS documentation and preventive care, case management or gerontological nursing certification, and experience across various virtual care programs.
