This is a fully remote position, open to applicants in United States.
• Evaluate patient needs via telephone utilizing nursing processes and established protocols.
• Identify the appropriate level of care, deliver education, create care plans, and communicate follow-up instructions.
• Collaborate with on-call providers for additional guidance when necessary.
• Prioritize patient interactions based on acuity and necessity.
• Utilize evidence-based practices to provide patient care.
• Implement measures to mitigate patient risk and enhance safety.
• Evaluate patient and family readiness for learning and customize education accordingly.
• Advocate for shared governance and engage in process improvement initiatives.
• Participate in professional development, attend required meetings, educational programs, and complete annual competencies.
• Effectively communicate and coordinate patient-centered care with clinicians and healthcare team members.
• Identify and report patient safety events and near misses; assist with implementing corrective actions.
• Employ ethical decision-making and function as an effective member of the patient care team.
• Offer age-appropriate care advice and disposition following departmental policies, procedures, and protocols.
• Schedule appointments in line with protocol recommendations.
• Coordinate medical and nursing management, including procedures and medication refills.
• Maintain and update clinical records in accordance with agency, state, and federal regulations.
• Document call encounters in the Electronic Medical Record during the call.
• Relay patient status and care-plan updates to physicians and interdisciplinary team members.
• Engage in team meetings and special projects assigned by leadership.
• Contribute to health-information resources, system efficiencies, and operational enhancements.
• Assist in interpreting departmental policies and advising staff on procedural changes.
• Registered Nurse license issued by the state where the team member practices.
• Associate's Degree (or equivalent knowledge) in Nursing.
• A minimum of 2 years of acute care experience, preferably within the last 4 years; experience in ED, Urgent Care, ICU, and some Med/Surg will be considered.
• Highly preferred 2+ years of experience in ED and/or Urgent Care.
• Strong critical thinking, sound judgment, and effective problem-solving abilities.
• Knowledge of healthcare specialties, levels of care, symptom identification, treatment recommendations, and established protocols.
• Exceptional verbal communication skills, encompassing empathy, respect, restatement, open-ended questions, active listening, and diplomacy.
• Capability to build rapport and maintain professional relationships with patients, staff, and physicians.
• Proficient in organizing and prioritizing work, managing multiple priorities, and maintaining a flexible schedule.
• Excellent customer service and follow-up skills.
• Proficiency with computers, internet, desktop software packages, and multi-line telephone systems.
• Ability to research and document calls across multiple systems.
• Required stable and secure internet connection.
• Must possess functional vision, touch, speech, and hearing.
• Must have a quiet space to make and receive phone calls.
• Ability to lift 15 lbs.
