Travel Nurse Practitioner / Physician Assistant

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Deliver thorough and personalized medical care to underserved patients, primarily focusing on those with Chronic Kidney Disease (CKD) progressing to End-Stage Renal Disease (ESRD).

• Travel within the designated area to perform in-home assessments, annual wellness exams, targeted screenings, and virtual consultations.

• Occasionally travel out of state for periods of up to two weeks as required.

• Provide counseling and education to patients and their families regarding benefits, available programs, preventive care, and prescribed treatments.

• Accurately document chief complaints, diagnoses, medical histories, system reviews, examinations, medications, allergies, assessments, and care plans.

• Collaborate with primary care providers, specialists, ancillary services, and the patient's Monogram physician to coordinate care.

• Ensure timely completion of documentation and paperwork.

• Uphold quality-of-care standards.

• Provide evidence-based care to minimize preventable hospitalizations while enhancing patient quality of life and satisfaction.

• Prescribe medications and order necessary tests.

• Measure and report patient quality outcomes on a daily basis.

• Address gaps in care through preventive monitoring, education, and coordination of diagnostic testing.

• Assist patients in enrolling to access educational videos.

• Engage in integrated care team meetings.

• Evaluate patients' health status, formulate and implement treatment plans, and assess follow-up status.

• Perform additional duties as assigned.

⛳️ Requirements

• Valid and unrestricted Registered Nurse and Nurse Practitioner licensure.

• Board certification relevant to licensure (NP: ANCC/AANP).

• Current and unrestricted DEA certificate.

• Willingness to travel up to 75% of the time.

• Ability to work independently and practice autonomously.

• Access to reliable transportation, a valid driver's license, and car insurance.

• Proficient in utilizing necessary medical instruments and equipment.

• Familiarity with computer-based data management systems, medical records, and point-of-interview technology.

• Strong verbal and written communication skills with retail and medical partners, patients, families, physicians, and community representatives.

• Knowledge of federal and state regulations, including HIPAA and OSHA.

• A minimum of two years of direct patient care experience is required.

• Bilingual in Spanish is advantageous but not mandatory.

• Experience in Managed Care/IPA/Health Plan environments.

• Background in conducting wellness visits, focused assessments, or similar comprehensive visits either virtually or in-home.
