Travel Nurse Practitioner

This is a fully remote position, open to applicants in Texas.

📋 Description

• Deliver personalized, empathetic medical care to underserved patients, focusing mainly on chronic kidney disease (CKD) through end-stage renal disease (ESRD).

• Travel across the designated area to perform in-home assessments, annual wellness exams, targeted screenings, and other visits both in patients' residences and virtually.

• Occasionally travel out of state for periods of up to two weeks as required.

• Provide counseling and education to patients and their families regarding benefits, programs, preventive care, and prescribed treatments.

• Record chief complaints, diagnoses, medical histories, reviews of systems, examinations, medications, allergies, assessments, and plans meticulously.

• Collaborate with primary care providers, specialists, and ancillary services to coordinate patient care effectively.

• Ensure timely completion of documentation and paperwork.

• Uphold quality-of-care standards consistently.

• Deliver evidence-based care aimed at minimizing avoidable hospitalizations and enhancing quality of life.

• Prescribe medications, order necessary tests, and work in conjunction with Monogram physicians.

• Daily monitor and report on patient quality outcomes.

• Address gaps in care through preventive monitoring education and by scheduling diagnostic testing.

• Assist patients in enrolling to access educational videos.

• Participate in integrated care team meetings.

• Evaluate patient health status, devise and implement treatment plans, and follow up on patient progress.

• Perform additional duties as assigned.

⛳️ Requirements

• Active and unrestricted licenses as a Registered Nurse and Nurse Practitioner.

• Board certification relevant to the appropriate licensure (NP: ANCC/AANP).

• Current and unrestricted DEA certificate.

• Willingness to travel up to 75% of the time.

• Capability to work independently without direct supervision and practice autonomously.

• Access to reliable transportation, a valid driver's license, and car insurance.

• Proficiency in using medical instruments and equipment needed for the role.

• Familiarity with computer-based data management systems and information technology.

• Understanding of medical records and point-of-interview technology.

• Strong verbal and written communication skills with retail and medical partners, patients, family members, physicians, and community representatives.

• Knowledge of federal and state regulations, including HIPAA and OSHA.

• A minimum of two years of direct patient care experience.

• Experience in Managed Care/IPA/Health Plan settings.

• Experience in conducting wellness visits, focused assessments, or similar comprehensive visits, either virtually or in-home.

• Bilingual in Spanish is advantageous; however, it is not a requirement.
