Territory Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Supervise a team of up to 15 Community Health Navigators.

• Guide the team in reaching monthly and quarterly performance targets.

• Manage outreach and member engagement through door-to-door visits and telephonic outreach.

• Ensure the timely completion of assessments and care plans, the delivery of high-quality care, and successful member graduation.

• Maintain readiness for audits and comply with HIPAA, documentation standards, and state and health plan regulatory requirements.

• Establish clear expectations, model effective behavior, and foster open communication.

• Support the field-based team by spending time on-site.

• Collaborate with community-based organizations, health plans, and other partners.

• Instruct and coach the team on escalation protocols, caseload management, safety expectations, and providing exceptional member care.

• Oversee daily territory operations, including individual performance and caseload evaluations.

• Monitor individual productivity, growth, member engagement, quality of care, and graduation rates.

• Offer continuous virtual and field-based coaching and development.

• Provide community-based opportunity resources in coordination with the Community Impact Partnership Program Director.

• Guide Community Health Navigators on SMART goals, interventions, and navigating member barriers and objections.

• Address issues raised by Community Health Navigators proactively and creatively.

• Support program growth, staffing requirements, and operational improvements.

• Recruit, manage, and mentor staff.

• Oversee Profit and Loss for the territory.

• Review and authorize payroll, expenses, and other compliance requirements for the territory.

• Adjust the schedule and be available on call during the designated workweek and other necessary hours.

• Travel regularly by car to client residences and additional locations.

⛳️ Requirements

• A minimum of 3-4 years of experience in managing personnel within health, social, or community services.

• High School Diploma or equivalent.

• Strong analytical abilities with a clear understanding of and comfort with data, dashboards, and reporting tools.

• Capability to manage a P&L.

• Adaptability to constant changes and ability to perform well under pressure in a fast-paced and challenging environment.

• Proficient in problem-solving, critical thinking, and effective collaboration.

• Knowledge of patient-centered care and whole-person care models.

• Ability to safely operate a motor vehicle and maintain a valid driver’s license and automobile insurance, as required by job responsibilities and applicable laws.

• Effective communication skills in person, by phone, and through electronic communication methods.

• Capability to sit, stand, walk, bend, reach, and use hands and fingers to perform essential job functions.

• Ability to carry and transport work-related materials or equipment weighing up to 10 pounds occasionally.
