Telephonic Care Coach

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Oversee members' health concerns using telephonic methods, strategies, and techniques.

• Recognize and address obstacles to effective care.

• Perform assessments and track patient care and progress towards targeted outcomes.

• Leverage data, discussions with members, and proactive care planning to enhance care.

• Direct members and their families to vital health and community resources.

• Implement professional concepts, standards, regulations, strategies, and operational guidelines.

• Make decisions on work methods and priorities while adhering to directives and established procedures.

• Report to the Manager of Care Management.

• Become a part of a committed team that promotes compassionate, patient-centered service.

⛳️ Requirements

• Bachelor's degree in a health-related discipline, current Illinois LPN licensure, or the capacity to obtain an IL LPN license.

• Two or more years of experience in case management, medical case management, care coordination, or healthcare support.

• Extensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook.

• Prior experience with electronic case note documentation.

• Experience documenting in various computer applications/systems.

• Background in health promotion, coaching, and wellness.

• Familiarity with community health and social service agencies, along with additional community resources.

• Ability to work Monday to Friday during operating hours of 8:00 AM to 7:00 PM CST, with core hours primarily from 8:00 AM to 5:00 PM CST.

• Minimum home internet speed of 25 Mbps download and 10 Mbps upload.
