This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Conduct virtual wellness consultations for adults and/or pediatric patients from their residences.

• Execute health assessments, health risk evaluations, medication reviews, cognitive screenings, and wellness services.

• Provide educational resources to patients regarding their health and wellness.

• Collaborate with the clinical in-home team (Sprinters) visiting each patient's residence.

• Assess vital signs and organize blood draws.

• Coordinate and oversee preventive care strategies, medication management, specialist referrals, community resources, and patient care.

• Record clinical observations.

• Review patient caseloads and prepare necessary technology at the start of each day.

• Manage daily schedules utilizing advanced technology.

• Deliver personalized, patient-centered care while upholding high-quality standards.

⛳️ Requirements

• Must be Board Certified as a Family Nurse Practitioner.

• Hold an active Family Nurse Practitioner license in Louisiana.

• Previous experience with wellness visits is required.

• Display high professionalism, integrity, accountability, confidentiality, care, and compassion.

• Willingness to engage in quick problem-solving and creative thinking.

• Proficient with technology and comfortable using laptops or mobile devices for documentation and HIPAA-compliant messaging applications.

• Excellent written and verbal communication abilities.

• Capable of working independently or collaboratively within a team setting.

• Must successfully pass a national background check and valid clinical license verification.

• Committed to providing high-quality health services.

• Required application details include states licensed as a Nurse Practitioner, experience in Medicare and/or Medicaid Annual Wellness Visits, Medicaid enrollment status, virtual Family Nurse Practitioner experience, notice period, prescriptive authority, and board certification.
