Telehealth/Virtualist – On-Demand Urgent Care

This is a fully remote position, open to applicants in New Jersey, +1 more state.

📋 Description

• Perform thorough virtual evaluations of patients, including in-depth medical histories and telehealth-compatible physical assessments.

• Request and analyze diagnostic tests when clinically justified.

• Address urgent care issues through telehealth services.

• Evaluate risks related to acute illnesses and decide on the necessity for in-person escalation.

• Collaborate with the interdisciplinary team to diagnose acute and episodic conditions.

• Create and prioritize differential diagnoses.

• Identify complications and arrange referrals to in-person medical care.

• Develop evidence-based treatment strategies for virtual urgent care.

• Prescribe medications, diagnostics, and interventions in line with telehealth clinical guidelines.

• Adjust care plans based on follow-up assessments, changing symptoms, and patient requirements.

• Record telehealth encounters in EPIC, documenting assessments, care plans, orders, and follow-up suggestions.

• Ensure documentation adheres to telehealth compliance, quality, and acuity-capture standards.

• Collaborate with clinical leaders, support staff, and healthcare-network partners to ensure continuity of care.

• Communicate effectively with patients and their families regarding virtual appointments, treatment plans, and follow-up actions.

• Engage in telehealth-focused training, education, and performance enhancement initiatives within the organization.

• Maintain continuing education units (CEUs), board certification, and professional memberships.

• Contribute to Jefferson’s virtual care framework.

• Carry out additional responsibilities as directed.

⛳️ Requirements

• DNP, MSN/MSRN, or PA equivalent as specified in the job summary.

• Master’s or Doctoral Degree required for Nurse Practitioners (NP) or Physician Assistants (PA).

• NP or PA must be board certified in Family Medicine.

• Experience in Emergency Room (ER) or Urgent Care settings is essential.

• Must reside in Pennsylvania, New Jersey, or Delaware.

• Hold an active license or be able to obtain licensure in Pennsylvania, New Jersey, and Delaware.

• An active Pennsylvania DEA is mandatory at the commencement of employment.

• Visa sponsorship is not available.

• Previous telehealth experience is preferred.

• Familiarity with high-acuity triage, virtual workflows, and remote diagnostic decision-making is favored.

• Required to maintain CEUs, board certification, and professional memberships.
