Telehealth Physician – Care Delivery

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct thorough telehealth initial consultations for enrolled members lasting 10-15 minutes.

• Integrate complex medical and social histories to identify key clinical priorities and risks.

• Develop clear and actionable clinical plans and handoffs for Community Health Workers, LCSWs, and care navigators.

• Accurately document findings and recommendations in the EMR to support ongoing care.

• Offer clinical direction regarding escalation pathways and program workflows during the pilot phase.

• Establish quick clinical rapport and trust with members and their caregivers.

⛳️ Requirements

• MD or DO with an active and unrestricted license.

• Board-certified or board-eligible in Family Medicine, Internal Medicine, Geriatrics, or a related primary care specialty is preferred.

• Significant experience working with Medicare/Medicaid populations or in complex chronic disease management.

• Proficient in telehealth assessments and capable of building rapport in a virtual setting.

• Strong clinical judgment, comfort with complexity and ambiguity, and a keen interest in care coordination.

• Comfortable operating in an iterative, startup-like pilot environment.
