This is a fully remote position, open to applicants in Washington.
• Establish and nurture relationships with community host sites, tribal health programs, county health departments, libraries, senior centers, and various local partners.
• Oversee event logistics in collaboration with host organizations, ensuring site accessibility assessments and effective communication on the day of the event.
• Manage outreach materials and communication channels, including flyers, event calendars, social media, local media, and partner newsletters.
• Identify local healthcare providers and specialty care facilities.
• Assist participants in obtaining referrals through warm handoffs, consent, and follow-up.
• Schedule and confirm telehealth appointments for participants.
• Manage provider availability and telehealth coverage schedules.
• Operate the telehealth platform, including device setup, readiness and connectivity checks, and providing real-time technical support.
• Maintain backup workflows in areas with low coverage.
• Monitor outreach activities, event participation, and referrals from scheduling to completion, including telehealth encounters, within BEM's reporting system.
• Engage in monthly coordination calls with AASCW and participate in internal team meetings.
• Travel to mobile clinic locations across the four-county area.
• 1–3 years of experience as a Community Health Worker or in a similar community outreach, social services, health navigation, telehealth, or healthcare scheduling role.
• Experience in relationship-building with diverse rural, tribal, and underserved communities.
• Proficient in troubleshooting technology, including tablets, hotspots, and video platforms, in field conditions.
• A valid driver's license and current automobile insurance.
• Reliable personal vehicle.
• Willingness to travel at least 60% of the time within a multi-county rural area.
• Overnight hotel accommodations may be necessary depending on event schedules.
• Strong communication skills in meetings, over the phone, and in person.
• Excellent organizational and time management abilities.
• Attention to detail and accuracy in high-volume tasks.
• Self-motivated, adaptable, and effective with limited supervision.
• Technological proficiency, including MS Office 365, scheduling software, and spreadsheets for data management.
• Strong interpersonal skills with a diverse range of individuals.
• Results-oriented under tight deadlines, capable of working independently and collaboratively within a team.
• Must successfully pass a background check.
• Bachelor's degree or higher is an advantage.
• Bilingual (Spanish or other languages) is a plus.
• Familiarity with HIPAA and patient privacy regulations is advantageous.
