Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in Arizona, +12 more states.

📋 Description

• Perform thorough health assessments via telehealth consultations

• Evaluate patients' medical histories, current medications, preventive care needs, and pertinent member data

• Accurately record HCC risk adjustment findings during consultations

• Identify and assist in closing care gaps related to HEDIS quality measures

• Utilize ICD-10 and CPT II codes to ensure complete and compliant documentation

• Provide clear care plans and follow-up instructions to members

• Ensure timely documentation that supports quality programs and value-based care workflows

• Operate within established processes to maintain consistent visit flow and documentation quality

• Enhance access to preventive and value-based care for Medicare and Medicaid beneficiaries

• Assist in ensuring accurate risk adjustment documentation and improved quality outcomes

⛳️ Requirements

• Active Nurse Practitioner licenses in a minimum of two eligible states: PA, WA, IA, KS, NJ, MT, WV, IN, WI, TX, TN, AZ, or MO

• Minimum of 1 year of experience in Family Medicine or Internal Medicine

• Experience working with Medicare and Medicaid populations

• Knowledge of HEDIS and risk adjustment workflows

• Enrollment as a provider for Medicare and Medicaid

• Capability to work no less than 24 hours per week

• Proficient in delivering care virtually and documenting thoroughly within a structured workflow

• Experience with ICD-10 and CPT II coding is required

• Experience in value-based care, Annual Wellness Visits, and HCC risk adjustment documentation is preferred
