Team Lead, Contract Care Coordination

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Supervise the daily operations of the Clinical Care Coordination team.

• Conduct regular supervision sessions and facilitate team meetings.

• Manage referral triage, case conferencing, documentation review, training compliance, and productivity oversight.

• Offer coaching, mentorship, and opportunities for professional growth.

• Track care coordination performance metrics, perform monthly audits, and provide internal reports to the Program Manager.

• Ensure accurate and timely tracking of case statuses for assigned referrals.

• Assist in the recruitment, onboarding, and training of clinical care coordinators and operational support personnel.

• Provide clinical care coordination services and manage caseload coverage as necessary.

• Support requests from the manager and assist with departmental operational tasks.

• Help with external provider consultation and coordination, including service continuation evaluations.

• Act as a point of contact for routine inquiries regarding services and referrals.

⛳️ Requirements

• A Master’s degree in Social Work, Psychology, or a related discipline is required.

• A Bachelor’s degree with a minimum of 3 years of leadership experience may be considered.

• At least two years of experience in providing behavioral health or social services within the military community is mandatory.

• Supervisory experience is preferred.

• Strong written, verbal, in-person, and virtual communication skills.

• Proficient in Microsoft Office 365 Applications, CRM Platforms, Zoom, basic Exploratory Data Analysis & Descriptive Statistics, and Adobe.

• Capacity to establish and achieve simultaneous short- and long-term objectives.

• Ability to effectively catalyze change within a team environment.
