This is a fully remote position, open to applicants in Tennessee.
• Assist members in enhancing their physical, behavioral, and social health and well-being.
• Aid members in securing and sustaining competitive, integrated employment opportunities.
• Navigate complex relationships to guarantee that members receive essential services.
• Conduct intake, enrollment, continuous assessments, and comprehensive face-to-face visits with members.
• Create and uphold Person-Centered Plans.
• Supervise and ensure the provision of overall services.
• Collaborate with members and their families to identify supports and services that promote health, employment, and goals for independent or community living.
• Work alongside clinical and social support teams to address needs related to physical and behavioral health, as well as long-term services.
• Evaluate members’ physical, behavioral, functional, psychosocial, financial, employment, and independent living statuses and needs.
• Implement authorization criteria for suitable home- and community-based services, ensuring that services are delivered and members’ needs are fulfilled.
• Travel to members’ residences throughout Davidson County.
• Registered Nurse (RN) preferred, with a preference for current Developmental Disabilities Nurses Association certification: Certified Developmental Disabilities Nurse (CDDN) for RNs or Developmental Disabilities Nurse (DDC) for LPNs, as applicable.
• Alternatively, a B.S. in social work, nursing, education, human services, healthcare profession, or a related field.
• Licensed Master Social Worker or Licensed Clinical Social Worker is preferred.
• Must meet federal standards for a Qualified Developmental Disabilities Professional (QDDP or QIDP).
• In lieu of a degree, a minimum of 5 years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver, along with completion of Personal Outcome Measures Introduction and Assessment Workshop trainings established by the Council on Quality and Leadership.
• At least 2 years of experience in the healthcare sector or an operations-type environment is required.
• A valid driver’s license is necessary.
• Various immunizations and/or related medical tests may be required.
• A digital literacy assessment is required.
• Proficiency in Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
• Ability to work independently with minimal supervision or as part of a team.
• Strong problem-solving skills and the ability to manage multiple assignments with critical deadlines.
• Demonstrated teamwork, organizational skills, reliability, strong work ethic, and ability to handle several projects and tasks simultaneously.
• Excellent verbal and written communication abilities.
• Strong interpersonal and organizational capabilities.
• Employees regularly using a company, personal, or rental vehicle must comply with driver safety program requirements, including maintaining a valid driver’s license, auto insurance compliance, and an acceptable motor vehicle record.
• Mileage reimbursement for travel to members’ residences.
• Full-time employment position.
• No on-call scheduling required.
• Standard work hours from 8:00 am to 5:00 pm.
