Supervisor, Care Management

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Provide direct supervision and oversight for a designated team of localized, multidisciplinary professionals assisting members with intellectual or developmental disabilities and traumatic brain injuries.

• Ensure staff utilize Trillium Connections workflows to conduct thorough care-management assessments and develop care plans as members' needs evolve.

• Foster a team-oriented, person-centered strategy for coordinating members' physical health, behavioral health, and social determinants of health.

• Guarantee that care coordination is executed effectively, efficiently, and in compliance with NCQA procedures.

• Oversee teams to promote integrated care through multidisciplinary collaboration involving members, caretakers/legal guardians, primary care providers, behavioral health and IDD providers, specialists, nutritionists, pharmacists, and others.

• Implement evidence-based and industry-standard practices to create processes, procedures, and workflows for efficient care coordination and management.

⛳️ Requirements

• Bachelor’s degree or licensure as a Registered Nurse, with five (5) years of experience in care management, case management, or care coordination for complex individuals with I/DD or TBI; OR a Master’s degree in a human services field or licensure as a Registered Nurse, along with three (3) years of relevant experience.

• Valid driver's license.

• Must live in North Carolina and within 75 miles of Trillium’s Midstate Region, covering Anson, Guilford, Montgomery, Randolph, and Richmond counties.

• Ability to travel within the catchment area as needed.

• Experience working with the Innovations Waiver is preferred.

• At least two (2) years of supervisory experience is preferred.

• All candidates are required to pass a drug test as a condition of employment.
