This is a fully remote position, open to applicants in United States.
β’ Facilitate remote order entry, verification of orders, and validation of medications.
β’ Conduct transitions-of-care medication reconciliations.
β’ Efficiently navigate various pharmacy information systems.
β’ Deliver drug information and therapeutic advice to healthcare providers.
β’ Evaluate the appropriateness of medications, including dosing adjustments for renal and hepatic functions, interactions, and compatibilities.
β’ Assist specialized populations such as pediatrics, geriatrics, pregnant or nursing patients, HIV patients, and those with renal or hepatic impairments.
β’ Stay informed about IV therapies, TPN, cardiac and narcotic drips, IV antibiotics, chemotherapy, and associated treatments.
β’ Accurately and promptly document clinical actions and interventions.
β’ Engage in communication with physicians and nursing staff.
β’ Ensure adherence to HIPAA regulations, pharmacy laws, and client policies.
β’ Operate across VPNs, automation systems, and various pharmacy information platforms.
β’ Participate in ongoing training related to clinical practices, PIS, and policy updates.
β’ Achieve productivity and quality benchmarks.
β’ Provide telepharmacy services to multiple hospitals across different states.
β’ A Bachelor's degree in Pharmacy is required; a PharmD is preferred.
β’ A minimum of 1 year of hospital pharmacy experience is highly preferred.
β’ An active Illinois or California pharmacist license in good standing is required.
β’ Must be willing to obtain additional state licenses for Iowa, Nebraska, and Hawaii.
β’ Active pharmacist licenses for Illinois, Nebraska, Hawaii, Iowa, and California are required in accordance with job title and licensure requirements.
β’ Strong clinical knowledge and comfort with multiple systems is essential.
β’ Availability to work evenings, overnight shifts, weekends, and holidays.
β’ Must have an internet connection through a cable broadband or fiber optic provider with a minimum of 10 Mbps download and 5 Mbps upload speed.
β’ Proficiency in using VPNs, automation systems, various pharmacy information systems, Google Suite/Microsoft Office, and Snagit.
β’ Familiarity with IV therapies, TPN, cardiac/narcotic drips, IV antibiotics, chemotherapy, medication dosing, interactions, and compatibilities is necessary.
β’ Capability to maintain required licenses, credentials, and system access.
β’ Must adhere to productivity and quality standards.
