Clinical Pharmacist

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee the daily management of clinical programs, ensuring the effective maintenance of databases and processes to schedule and assist patients while adhering to program design.

• Provide support in clinical disease management by implementing care plans.

• Perform telephonic assessments and health coaching for patients with chronic conditions.

• Foster patient and prescriber engagement in clinical program initiatives.

• Assist with the clinical and operational needs of various business units as necessary.

• Educate healthcare providers on claims and benefits procedures.

• Create reports that reflect work outcomes for patients and clients.

• Review relevant literature and develop thorough responses to drug information inquiries.

• Aid in the establishment of policies, procedures, and internal processes.

• Conduct drug utilization reviews and implement necessary interventions.

• Collaborate with the Manager of Clinical Programs to develop and implement clinical programs.

• Carry out comprehensive medication reviews and consultations regarding drug therapy.

• Work alongside clinical program staff, including nurses and health coaches, to enhance population management and care coordination.

• Engage in communication with patients and providers, including making outbound calls.

• Adhere to Federal and State pharmacy regulations, along with company policies and procedures.

• Report directly to the Manager of Clinical Programs.

⛳️ Requirements

• Doctor of Pharmacy degree from an accredited institution.

• Active pharmacist license in good standing in the state of practice.

• Clear and active Florida license as mandated by the profession.

• Required proficiency in Microsoft Word, Excel, and Outlook.

• Minimum of two years of clinical experience preferred.

• 2-3 years of experience in a managed care environment preferred.

• Previous experience in delivering clinical presentations and/or progress updates is preferred.

• Familiarity with formalized projects or enhancements in clinical operational processes is preferred.

• Experience in clinical and analytical problem-solving, along with expertise in drug information.

• Preferred proficiency in Power Point and Access.

• Capability to present clinical program initiatives to internal departments or external clients.

• Knowledge of the scope of clinical services permitted by the profession, as well as relevant laws and regulations.

• Ability to assess drug therapy appropriateness, effectiveness, safety, and adherence.

• Strong communication skills with healthcare providers to ensure continuity of care.

• Dependability and a commitment to maintaining confidentiality are essential.
