πŸ“‹ Description

β€’ Conduct telephonic clinical consultations in line with Lumicera's Patient Management Program.

β€’ Perform patient evaluations and reconcile medications.

β€’ Identify barriers to adherence, potential drug interactions, medication-related issues, and opportunities for clinical intervention.

β€’ Provide education on disease management, medication administration, storage requirements, treatment expectations, and mitigation of side effects.

β€’ Address clinical inquiries from patients and caregivers.

β€’ Escalate patient-specific clinical issues through established protocols.

β€’ Document consultations, assessments, interventions, and clinical recommendations within pharmacy systems.

β€’ Uphold departmental productivity, quality, and service-level standards.

β€’ Support quality improvement efforts by identifying, documenting, and reporting medication errors and patient safety issues.

β€’ Stay updated on specialty medications, treatment guidelines for diseases, and emerging therapies.

β€’ Comply with federal and state laws, pharmacy regulations, accreditation standards, and organizational policies.

β€’ Perform additional duties as assigned.


⛳️ Requirements

β€’ Must be a graduate of an ACPE Accredited School of Pharmacy with a bachelor's degree in Pharmacy or PharmD.

β€’ Current, unrestricted licensure as a Registered Pharmacist in the state of residence, or the ability to obtain reciprocity within 4 months, may be required based on state laws or client contracts.

β€’ Required experience in counseling patients and caregivers regarding medication therapy, treatment expectations, and disease management.

β€’ Required experience in documenting patient assessments, clinical interventions, and medication-related recommendations following pharmacy practice standards.

β€’ Required experience in evaluating medication regimens to identify potential drug interactions, adherence barriers, and other medication-related concerns.

β€’ Must participate in, adhere to, and support compliance program objectives.

β€’ Ability to consistently interact cooperatively and respectfully with colleagues.

β€’ Must reside in one of the states eligible for remote work as listed in the notification.
