Specialty Care Navigator, Trauma

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephonic screenings, brief interventions, and referrals for members to specialized trauma treatment providers.

• Employ active listening, psychoeducation, and motivational interviewing techniques to comprehend each member’s requirements and assist them in goal-setting.

• Apply Judith Herman’s phase-based trauma treatment model to provide stabilization support to members as they engage in their care journey.

• Use motivational interviewing to support members in their engagement with care.

• Assess and determine the appropriate level of care, facilitate referrals, and coordinate with a Spring Health provider.

• Offer consultations regarding mental health issues along with family and relationship concerns, tailored to member needs.

• Provide immediate support and safety planning for members facing safety risks or crises, when necessary.

• Utilize written communication skills to correspond with members and providers via email.

• Maintain accurate documentation, records, and follow-ups while tracking progress across various systems, including EHR, within designated timeframes.

• Identify and troubleshoot barriers or disruptions to care.

• Collaborate with multi-disciplinary teams such as care support, provider operations, and benefits/billing to ensure a seamless member experience.

• Assist other specialty care teams, including SUD, Mood and Anxiety, and Eating Disorders, as needed.

• Engage in staff meetings, case consultations, and training sessions.

• Adhere to Spring Health policies while maintaining confidentiality, compliance, and ethical standards.

⛳️ Requirements

• Valid mental health license required: LCSW, LMFT, LPC.

• At least 2+ years of clinical care experience dealing with PTSD and Complex Trauma.

• Proficiency in motivational interviewing, diagnostic assessment, and triage.

• Familiarity with phase-based trauma treatment models and evidence-based practices for PTSD (e.g., Prolonged Exposure, Cognitive Processing Therapy, EMDR, ART, etc.).

• Exceptional listening, written, and verbal communication skills.

• Tech-savvy: must be adept at using different computer platforms, learning new systems, and efficiently handling tech-related tasks.
