This is a fully remote position, open to applicants in Texas.
• Collaborate with providers and VillageHealth Nurses to deliver ongoing support and effective communication to patients with Chronic Kidney Disease (CKD).
• Serve as the primary point of contact for coordinating resources for VillageHealth CKD patients.
• Conduct outbound calls to patients to assess clinical needs and connect them with suitable resources.
• Make outbound calls to providers to schedule appointments for patients or to follow up on their care.
• Respond to inbound calls from patients, providers, and Village resources.
• Follow up with patients to ensure their needs are addressed and arrange future check-ins.
• Collaborate with clinical resources and providers to guarantee seamless continuity of care.
• Monitor patient hospitalizations and follow up as needed with a VillageHealth Nurse.
• Inform patients about location details and appointment times.
• Provide educational materials to patients and send letters to primary care physicians, nephrologists, and specialists for new enrollments or appointments.
• Assist in the referral process.
• Perform additional duties as assigned.
• High school diploma or equivalent qualification.
• Minimum of two (2) years of experience in a professional office setting.
• Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
• Exceptional verbal and written communication skills.
• Capability to manage confidential information and issues with discretion and sound judgment.
• Must fulfill applicable employment eligibility requirements.
