Social Worker, MSW – Part-time, Flexible Day Shift

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Contribute to improving population health outcomes through the principles of value-based care.

• Collaborate with and act as a specialist consultant for Population Health Registered Nurses, healthcare providers, and practice personnel.

• Engage in management strategies that enhance desired health results.

• Advocate on behalf of patients, their families, and significant others.

• Act as a communication bridge between healthcare providers, patients, care team members, skilled nursing facilities, home health services, hospital staff, and community partners/resources.

• Assist patients in accessing community-based program resources.

• Empower patients to effectively navigate their medical care and community assistance options.

⛳️ Requirements

• A Master’s degree in Social Work is REQUIRED.

• A minimum of 2 years' experience in direct patient care and case management is REQUIRED.

• Experience with the frail or elderly population is preferred.

• Familiarity with Medicare, Medicaid, and CMS regulations is preferred.

• Candidates must live within a 30-minute driving radius of Harrisonburg, Rockingham, Mt. Jackson, Broadway, Staunton, or Elkton, VA.

• Travel may be necessary based on patient requirements or assigned cases.
