Social Worker

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Deliver approximately one hour of professional supervision weekly and offer consultation to the ADH Case Manager as required.

• Provide professional insights concerning psychosocial assessments, care plans, client-centered objectives, interventions, referrals, and follow-ups.

• Consult on intricate, high-risk, or challenging scenarios involving clients and their families, recommending suitable interventions.

• Assess social work documentation and practices for precision, quality, confidentiality, and adherence to regulations.

• Facilitate coordination of social work services among clients, families, caregivers, healthcare providers, community organizations, and interdisciplinary team members.

• Offer guidance regarding community resources, healthcare services, social service agencies, benefits, and referrals.

• Assist in case management and care coordination for older adults, individuals with disabilities, and other at-risk populations.

• Provide insights on Massachusetts Adult Day Health, Department of Public Health, MassHealth, and other regulatory stipulations.

• Ensure complete documentation, regulatory compliance, and audit preparedness.

• Identify and relay psychosocial concerns, safety hazards, compliance issues, and recommendations to the ADH Program Director and interdisciplinary team.

• Advocate for ethical practice, client rights, confidentiality, dignity, and culturally sensitive care.

• Act as a professional resource concerning psychosocial needs, social determinants of health, community resources, and care coordination.

• Collaborate effectively with nursing, medical, rehabilitation, behavioral health, case management, and other interdisciplinary team members.

• Stay informed about Massachusetts regulations, healthcare programs, community resources, and social work practices.

⛳️ Requirements

• A Licensed Clinical Social Worker (LICSW) credential is mandatory.

• Proven expertise in professional social work practices, including psychosocial assessment, care planning, case management, referrals, advocacy, and client support.

• Familiarity with community-based healthcare and social service frameworks.

• Understanding of Massachusetts Adult Day Health regulations and relevant healthcare and social service laws.

• Strong grasp of confidentiality, professional ethics, documentation standards, and client rights.

• Exceptional written and verbal communication abilities.

• Proficient interpersonal, organizational, critical-thinking, and problem-solving skills.

• Capacity to provide constructive professional feedback and guidance to Case Management staff.

• Ability to make sound professional judgments in complex client and family situations.

• Commitment to maintaining confidentiality and managing sensitive client information appropriately.

• Demonstrated dedication to compassionate, person-centered, trauma-informed, and culturally responsive care.

• Ability to collaborate with clients, families, caregivers, providers, community organizations, and interdisciplinary team members.

• Basic computer skills and capacity to review and assess electronic health records and documentation.

• Ability to remain seated for extended periods while using a computer, monitor, keyboard, and mouse.

• Capacity to visually examine electronic and written documentation.

• Ability to fulfill the essential functions of the position with or without reasonable accommodation.

• A Master of Social Work (MSW) degree is preferred.

• Experience working with older adults, individuals with disabilities, medically complex individuals, or other vulnerable groups is preferred.

• Prior experience in social work supervision, clinical consultation, case management, or professional oversight is preferred.

• Familiarity with Adult Day Health (ADH) programs, Massachusetts Department of Public Health (DPH) requirements, MassHealth Senior Care Options (SCO), Aging Services Access Points (ASAPs), community-based healthcare, long-term services and supports (LTSS), care coordination and interdisciplinary team models, older adults and individuals with disabilities, and community resources and social service systems is preferred.
