Social Worker

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Deliver crisis intervention and assist families in navigating the complexities and effects of medical situations.

• Evaluate the ongoing needs and responses of patients and families to healthcare services.

• Conduct interviews with patients and plan activities that address their social and emotional needs, promoting psychological, emotional, and physical recovery.

• Carry out telephonic outreach, care coordination, advocacy, and connect members with community and healthcare resources.

• Provide support to elderly and disabled members facing complex medical, behavioral, and social challenges.

• Collaborate with members, providers, caregivers, and community organizations to overcome barriers to care and enhance health outcomes.

• Coordinate with external agencies and home care nursing groups for care following discharge.

• Perform home visits as necessary.

⛳️ Requirements

• An active Tennessee Social Work license is required.

• A Master's degree in Clinical Licensure or Education is necessary.

• One of the following unrestricted Tennessee licenses: LCSW or LMSW.

• A valid Driver's License is mandatory.

• A minimum of 3 years of experience in care coordination for individuals receiving long-term care and/or home and community-based services is required.

• Various immunizations and/or related medical tests may be necessary.

• Experience in skilled nursing facilities (SNF), acute inpatient settings, discharge planning, community resource coordination, or hospice care is especially valued.

• Must possess strong attention to detail, handle ambiguity effectively, and demonstrate excellent strategic thinking and creativity skills.

• Ability to communicate effectively with both internal and external partners/customers.

• Understanding of the effects of cultural/socioeconomic status on healthcare and mental healthcare services.

• Must be capable of performing home visits if required.

• Employees who frequently operate a company, personal, or rental vehicle must participate in the BCBST Driver Safety Program, maintain a valid driver's license and compliant auto insurance, and have an acceptable motor vehicle record.

• CCM certification is preferred but not mandatory.
