This is a fully remote position, open to applicants in California.

📋 Description

• Assist the Care Management team in coordinating patient care.

• Collaborate closely with fellow healthcare team members.

• Execute complex tasks related to the overall functions and activities identified during the initial and ongoing assessment and management of patients.

• Facilitate patient-centered, holistic care under the guidance of the Regional Director of Care Management.

⛳️ Requirements

• Minimum of 5 years of experience in a hospital, clinical setting, or healthcare-related field.

• At least 3 years of exceptional customer service experience.

• 3 years of clerical and computer proficiency.

• 3 years of understanding of insurance billing and managed care.

• Preferred: Some college-level coursework or a high school diploma/GED equivalent, especially in Medical Terminology or Medical Assistance coursework.

• CHAA certification is preferred.

• CHAA certification is mandatory within 6 months (180 days) of hire if employed in California.

• 3 years of experience with EPIC Case Management and ADT Patient Access is preferred.
