This is a fully remote position, open to applicants in Texas.
β’ Execute care management responsibilities to evaluate and coordinate medical and supportive services throughout the continuum of care for complex/high-acuity populations.
β’ Create and continually evaluate individualized long-term care plans/service plans.
β’ Work collaboratively with care management teams, healthcare providers, specialists, and community resources.
β’ Facilitate communication between members/families/caregivers and care provider teams to guarantee appropriate person-centered care.
β’ Track member status, complications, clinical symptoms, care-plan advancement, and condition changes.
β’ Analyze member data, referrals, and intake assessments to enhance quality and formulate care plans.
β’ Gather, document, and preserve member information and care management activities to ensure regulatory and clinical compliance.
β’ Provide education to members and families/caregivers regarding disease processes, care gaps, care options, referrals, benefits, and provider instructions.
β’ Serve as a liaison and advocate for members between families, physicians, facilities, and agencies.
β’ Organize community resources and service authorizations, including meals, employment, housing, foster care, transportation, and daily living activities.
β’ May conduct home or other site visits to evaluate needs and collaborate with resources.
β’ Partner with leadership to enhance the quality of care and promote cost-effective service delivery.
β’ May mentor new clinical hires and assist with onboarding, competency evaluations, coaching, and readiness.
β’ Perform additional duties as assigned.
β’ Graduate from an Accredited School of Nursing or possess a Bachelor's degree along with 4β6 years of relevant experience.
β’ Bachelor's degree in Nursing is preferred.
β’ RN - Registered Nurse - State Licensure and/or Compact State Licensure is required, or NP - Nurse Practitioner - Current State's Nurse Licensure is required.
β’ For Superior: Resource Utilization Group (RUG) certification must be acquired within 90 days of employment.
β’ For Arkansas Total Care plan: a driver's license, child and adult maltreatment checks, and a drug screen are required.
β’ For Arkansas Total Care plan: must live in AR or a bordering city.
β’ For Arkansas Total Care plan: 5% travel is required.
