This is a fully remote position, open to applicants in California.
• Oversee the execution of clinical performance and care models across assigned care teams and regions.
• Collaborate with operational, nursing, advanced practice providers, physicians, and cross-functional leaders to ensure consistent and high-quality care delivery.
• Promote uniformity in clinical decision-making, escalation protocols, and documentation standards.
• Educate NPs and care team members on physiology, pathology, and care guidelines.
• Implement protocols, care pathways, and best practices into operational workflows.
• Identify and minimize unwarranted variations in care delivery.
• Focus on clinical aspects such as kidney disease management, comorbidity treatment, hospitalization prevention, transitions of care, and optimal treatment initiation.
• Provide both direct and indirect leadership to clinicians and interdisciplinary teams.
• Coach teams on complex clinical scenarios and management of high-risk patients.
• Support the onboarding, development, performance management, and continuous growth of clinicians.
• Enhance workflows, clarify roles, coordinate handoffs, and refine care team processes.
• Utilize performance data, clinical insights, and feedback from frontline staff to enhance operational reliability.
• Aid in scalable panel management, risk stratification, care coordination, and follow-up after discharge.
• Contribute clinical insights to decision support tools, documentation workflows, registries, and technology-enabled care processes.
• Collaborate with product, analytics, informatics, and operational teams to enhance visibility into patient risk, care gaps, and clinical performance.
• Analyze high-utilizer patient cases for opportunities to improve care delivered by NPs.
• Take ownership of clinical performance across designated care teams and markets.
• Drive enhancements in quality, patient outcomes, team performance, and execution of care models.
• Act as a reliable partner to operational, nursing, and enterprise clinical leaders.
• Influence and lead in a highly matrixed environment while nurturing patient-centered, accountable teams.
• MD, DO, NP, or PA is required.
• An active, unrestricted clinical license is mandatory.
• Extensive clinical experience in caring for patients with complex chronic diseases.
• Experience in kidney care, value-based care, primary care, hospital medicine, or population health is highly preferred.
• Proven experience in people leadership, including coaching, team development, and performance management.
• Background in leading interdisciplinary clinical teams in a matrixed setting.
• Strong expertise in value-based care models and their execution.
• Exceptional ability to leverage clinical judgment, data, and influence to scale improvements in outcomes.
• Excellent skills in communication, collaboration, and change leadership.
• Experience in kidney care, CKD/ESKD care models, or integrated care for patients with multiple chronic conditions is preferred.
• Experience in leading clinicians and managers across multi-site or scaled care delivery models is preferred.
• Familiarity with population health strategy, clinical operations, and care model transformation is preferred.
• Experience in designing or implementing clinical protocols, workflows, or decision support tools is preferred.
