This is a fully remote position, open to applicants in United States.
• Engage in proactive outreach and follow-up after hospitalizations to link members with behavioral health services.
• Organize appointments and facilitate access to benefits and community resources.
• Foster trusted relationships with members to evaluate their needs and identify suitable providers and resources.
• Tackle barriers to care and escalate risk concerns to clinical teams when necessary.
• Collaborate with clinicians, Provider Success, and internal teams regarding follow-up for high-risk members, provider access, program enrollment, and continuity of care.
• Inform members, providers, and facilities about available services and care-management protocols.
• Address access challenges and bridge care gaps.
• Ensure accurate and timely documentation while managing sensitive member information in accordance with HIPAA and organizational standards.
• Efficiently navigate multiple systems, communicate effectively, achieve performance targets, and contribute to a collaborative, member-focused team environment.
• A Bachelor's degree in Social Work, Psychology, or a related healthcare field; OR a high school diploma with 4+ years of experience in a healthcare, managed care, behavioral health, or customer service environment.
• A minimum of 2 years of experience in a healthcare, managed care, behavioral health, or customer service setting for candidates following the bachelor's-degree pathway.
• Strong computer skills, including the ability to work across multiple systems simultaneously and accurately input or retrieve data.
• Capability to simultaneously talk and type while navigating systems during real-time member interactions.
• Familiarity with behavioral health or medical terminology.
• Excellent problem-solving, critical-thinking, and organizational abilities.
• Proficient in prioritizing, planning, and multitasking while managing competing priorities.
• Ability to pass background checks upon hiring and throughout employment, which includes criminal, employment, education, healthcare sanctions, prohibited parties, and a 10-panel drug screening.
• Proven ability to quickly establish trust and motivate members.
• Skillful in managing difficult conversations and sensitive subjects.
• Proficient in de-escalation techniques, reflective listening, and responding effectively to member needs.
• Ability to problem-solve and provide timely solutions.
• High-speed internet service with a minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds, required prior to starting.
