Care Coordinator

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Collaborate with members and their families to identify necessary supports and direct services for health, employment, independent living, and community living objectives.

• Work together with clinical and social support team members to meet the physical, behavioral health, and long-term service requirements of members.

• Perform comprehensive, unbiased face-to-face visits in home or community settings.

• Evaluate the physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs of members.

• Utilize authorization criteria to approve suitable home- and community-based services.

• Ensure that authorized services are executed and that members' needs are adequately addressed.

• Handle requests and expectations from both internal and external stakeholders.

• Travel to members’ residences for visits.

⛳️ Requirements

• Minimum of 3 years of experience in a clinical environment.

• At least 2 years of clinical experience is required.

• Must be a registered nurse with an active license in Tennessee, or hold an active license in the employee’s state of residence if participating in the Nurse Licensure Compact Act.

• Alternatively, a Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license is acceptable.

• Outstanding customer service abilities.

• Residency within Shelby County, Tennessee is required.

• Availability for an 8:00 am–5:00 pm EST work schedule.

• Possession of a valid driver’s license.

• TB Skin Test is necessary for coordinators working in the field.

• Basic proficiency in Microsoft Office and email applications.

• Strong time management capabilities.

• Excellent verbal and written communication skills.

• Robust interpersonal and organizational skills.

• Compliance with BCBST Driver Safety Program requirements is mandatory when operating a company, personal, or rental vehicle, which includes a valid driver’s license, compliant auto insurance, and an acceptable motor vehicle record.

• Completion of a digital literacy assessment is required.

• Various immunizations and/or related medical tests may be necessary.
