Senior Care Specialist, Registered Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform telephonic health assessments for members of Medicare, Medicaid, and/or group health plans.

• Create actionable, personalized, and measurable care plans tailored to the needs and recognized health risks of members.

• Build health coaching relationships and offer education, motivation, and support for managing chronic conditions.

• Utilize clinical nursing judgment to evaluate member needs and coordinate interventions in acute situations.

• Assist with transitions of care and safe discharge planning, which includes the coordination of DME, home health services, provider appointments, and other resources.

• Aid members and caregivers in navigating healthcare benefits, services, and care options.

• Evaluate physical, environmental, and psychosocial factors that influence members' health and well-being.

• Collaborate with multidisciplinary teams, such as social workers, dietitians, pharmacists, providers, and other healthcare professionals.

• Encourage member self-management by providing condition-appropriate education and resources.

• Coordinate care and condition management activities among members, caregivers, providers, and care teams.

• Accurately and promptly document member interactions, assessments, care plans, interventions, and outcomes in relevant systems.

• Assist members with chronic and complex health issues using a Whole Person Care approach.

⛳️ Requirements

• Current, unrestricted Registered Nurse (RN) license in the state of residence.

• Valid Compact RN license allowing practice in applicable compact states.

• At least 2 years of clinical nursing experience; 3 or more years is preferred.

• Strong clinical judgment, critical thinking, and problem-solving abilities.

• Capability to work independently and apply appropriate nursing judgment in a remote setting.

• Intermediate to advanced proficiency in computer skills, including Microsoft Word, Excel, and Outlook.

• Ability to efficiently navigate various computer systems and electronic medical records.

• Excellent verbal and written communication skills.

• Preferred experience working with elderly and/or chronically ill populations.

• Preferred experience in care coordination, case management, or managed care.

• Preferred experience with Medicare and/or Medicaid populations.

• Preferred experience in chronic condition management and patient education.

• Preferred experience in telephonic nursing, health coaching, or remote care management.

• Preferred experience as part of a multidisciplinary healthcare team.
