Senior Care Specialist, Registered Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform telephonic health evaluations for members of Medicare, Medicaid, and/or group health plans.

• Create actionable, personalized, and quantifiable care plans tailored to member needs and recognized health risks.

• Build health coaching relationships while providing education, motivation, and support pertaining to chronic condition management.

• Utilize clinical nursing judgment to evaluate member needs and coordinate suitable interventions for urgent situations.

• Assist with care transitions and secure discharge planning, which includes coordinating durable medical equipment, home health services, provider appointments, and other essential resources.

• Aid members and caregivers in navigating healthcare benefits, services, and available care alternatives.

• Evaluate physical, environmental, and psychosocial factors that impact members' health and overall well-being.

• Collaborate with multidisciplinary teams, including social workers, dietitians, pharmacists, providers, and other healthcare professionals.

• Encourage member self-management through education and resources tailored to individual health conditions and objectives.

• Coordinate care and condition-management activities among members, caregivers, providers, and the care team.

• Accurately and promptly document member interactions, assessments, care plans, interventions, and outcomes in relevant systems.

• Operate via phone using a Whole Person Care approach to assist members with chronic and complex health conditions.

⛳️ Requirements

• Active, unrestricted Registered Nurse (RN) license in the state of residence.

• Active Compact RN license allowing practice in applicable compact states.

• At least 2 years of clinical nursing experience; 3 or more years is preferred.

• Strong clinical judgment, critical thinking, and problem-solving abilities.

• Capability to work independently and apply appropriate nursing judgment in a remote setting.

• Intermediate to advanced proficiency in computer skills, including Microsoft Word, Excel, and Outlook.

• Ability to navigate multiple computer systems and electronic medical records efficiently.

• Excellent verbal and written communication skills.

• Preferred experience in caring for elderly and/or chronically ill populations.

• Preferred experience in care coordination, case management, or managed care.

• Preferred experience with Medicare and/or Medicaid populations.

• Preferred experience in chronic condition management and patient education.

• Preferred experience in telephonic nursing, health coaching, or remote care management.

• Preferred experience working as part of a multidisciplinary healthcare team.
