Senior Care Specialist, Registered Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform telephonic health assessments for members of Medicare, Medicaid, and/or group health plans.

• Create actionable, personalized, and measurable care plans tailored to members' needs and identified health risks.

• Build health coaching relationships while providing education, motivation, and support for chronic condition management.

• Utilize clinical nursing judgment to evaluate member needs and coordinate interventions for urgent situations.

• Aid in transitions of care and ensure safe discharge planning, including coordination of durable medical equipment (DME), home health services, provider appointments, and other essential resources.

• Assist members and their caregivers in navigating healthcare benefits, services, and care alternatives.

• Evaluate physical, environmental, and psychosocial factors that impact members’ health and well-being.

• Collaborate with social workers, dietitians, pharmacists, providers, and various healthcare professionals.

• Encourage member self-management through education and customized resources.

• Coordinate care and condition-management efforts among members, caregivers, providers, and care team members.

• Accurately and promptly document member interactions, assessments, care plans, interventions, and outcomes in relevant systems.

⛳️ Requirements

• Must possess an active, unrestricted Registered Nurse (RN) license in the state of residence.

• Active Compact RN license allowing practice in applicable compact states.

• At least 2 years of clinical nursing experience required; 3 or more years preferred.

• Strong clinical judgment along with critical-thinking and problem-solving skills.

• Ability to work autonomously and demonstrate appropriate nursing judgment in a remote setting.

• Intermediate to advanced computer skills, including proficiency in Microsoft Word, Excel, and Outlook.

• Ability to efficiently navigate multiple computer systems and electronic medical records.

• Excellent verbal and written communication skills.

• Experience with elderly and/or chronically ill populations is preferred.

• Background in care coordination, case management, or managed care is preferred.

• Experience working with Medicare and/or Medicaid populations is preferred.

• Familiarity with chronic condition management and patient education is preferred.

• Experience in telephonic nursing, health coaching, or remote care management is preferred.

• Experience as part of a multidisciplinary healthcare team is preferred.
