Senior Care Specialist, Registered Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform telephonic health assessments for members of Medicare, Medicaid, and/or group health plans.

• Create actionable, personalized, and measurable care plans based on the needs and identified health risks of members.

• Build health coaching relationships while providing education, motivation, and support for managing chronic conditions.

• Utilize clinical nursing judgment to evaluate member needs and coordinate interventions in acute situations.

• Assist in transitions of care and safe discharge planning, which includes coordinating DME, home health services, provider appointments, and other resources.

• Aid members and caregivers in navigating healthcare benefits, services, and care alternatives.

• Evaluate physical, environmental, and psychosocial factors that influence members' health and well-being.

• Collaborate with multidisciplinary teams, such as social workers, dietitians, pharmacists, providers, and other healthcare professionals.

• Encourage member self-management by providing individualized education and resources.

• Organize care and condition-management activities among members, caregivers, providers, and care teams.

• Accurately and promptly document member interactions, assessments, care plans, interventions, and outcomes in relevant systems.

⛳️ Requirements

• Must hold an active, unrestricted Registered Nurse (RN) license in the state of residence.

• Active Compact RN license allowing practice in applicable compact states is required.

• A minimum of 2 years of clinical nursing experience is necessary; 3 or more years is preferred.

• Strong clinical judgment alongside critical-thinking and problem-solving abilities is essential.

• Capability to work independently and apply appropriate nursing judgment in a remote setting.

• Intermediate to advanced proficiency in computer skills, including Microsoft Word, Excel, and Outlook.

• Efficiently navigate multiple computer systems and electronic medical records.

• Excellent verbal and written communication skills are required.

• Preferred experience includes caring for elderly and/or chronically ill populations.

• Preferred background in care coordination, case management, or managed care.

• Experience with Medicare and/or Medicaid populations is preferred.

• Experience in chronic condition management and patient education is preferred.

• Prior experience in telephonic nursing, health coaching, or remote care management is preferred.

• Experience collaborating within a multidisciplinary healthcare team is preferred.
