Senior Care Manager, RN

This is a fully remote position, open to applicants in Oklahoma.

📋 Description

• Evaluate, plan, and execute intricate care management initiatives.

• Create tailored care plans/service plans for members.

• Regularly review care plans and collaborate with providers and community resources.

• Coordinate efforts among members, families/caregivers, and care provider teams to ensure person-centered care.

• Monitor member status, changes in condition, and progress towards care objectives.

• Identify obstacles and interventions for escalated cases.

• Analyze member data, referrals, and intake assessments to enhance care quality and operational performance.

• Conduct outreach via phone, digital platforms, home visits, and other locations.

• Work in partnership with healthcare providers to facilitate services and treatments.

• Record member information and care management activities to comply with regulatory standards.

• Educate members and caregivers about disease processes, care gaps, referrals, care options, and benefits.

• Collaborate with leadership to enhance cost-effective care delivery.

• Mentor and coach new clinical hires during their onboarding process.

• Guide preceptees within a Managed Care Organization while providing coaching and shadowing opportunities.

• Complete competency assessments and monitor readiness for Service Coordination.

• Work with People Leaders and cross-functional teams to evaluate readiness.

• Perform additional assigned tasks and adhere to policies and standards.

⛳️ Requirements

• Must be a resident of Oklahoma.

• Active RN license is mandatory.

• RN state licensure and/or compact state licensure is required.

• Degree from an accredited nursing school or a Bachelor's degree in Nursing.

• 4–6 years of relevant experience.

• Experience with children, foster care, and/or the juvenile justice system is preferred.

• Proficient with computers.

• Exceptional communication and customer service abilities.

• Experience managing a complex medical caseload.

• Strong assessment skills.

• Availability from Monday to Friday, 8:00 AM to 5:00 PM.
