This is a fully remote position, open to applicants in United States.
• Coordinate and assist with non-clinical patient care tasks
• Ensure that patients receive prompt services, follow-ups, education, and support in navigating their care
• Address obstacles and escalate issues as necessary
• Act as a resource for care coordination and take the lead within the team
• Provide training, guidance, workflow support, and operational oversight
• Contribute to process enhancements and effective service delivery
• Maintain precise patient records and documentation
• Assist with reporting and quality initiatives
• Ensure adherence to organizational, regulatory, and HIPAA standards
• Support the care delivery team for individuals with chronic and complex conditions
• Foster high-quality service and a positive experience for patients
• High School Diploma or GED
• A minimum of 3 years' experience in healthcare, care coordination, medical office operations, patient navigation, or a similar healthcare support position
• Strong comprehension of healthcare workflows, referrals, management of medical records, and patient support processes
• Familiarity with medical terminology
• Experience utilizing electronic medical record (EMR) systems and healthcare technology platforms
• Excellent organizational, problem-solving, and time-management abilities
• Must be able to pass a background check upon hiring and throughout employment, including specified criminal, employment, education, sanctions, watchlist, and drug-screen checks
• Capacity to juggle multiple priorities in a dynamic environment
• Ability to work independently while effectively collaborating with multidisciplinary teams
• Dedication to patient-centered care, confidentiality, and adherence to HIPAA regulations
• High-speed internet service with a minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds, must be installed before starting
• A quiet workspace with minimal background noise for calls
• Capability to remain stationary and seated for extended periods, communicate, examine information, perform repetitive motions, interpret data, resolve problems, make decisions, organize and plan, and maintain a positive and professional demeanor
