This is a fully remote position, open to applicants in United States.
• Provide remote, patient-focused case management support and education to designated patients.
• Establish and nurture therapeutic relationships with assigned patients throughout the duration of the program.
• Offer patient-centered education concerning treatments, disease states, and prescribed products.
• Manage an assigned caseload through scheduled outreach and adherence monitoring.
• Accurately record activities in program databases.
• Facilitate interactions between patients and internal or external clinical partners.
• Tailor interactions based on individual patient needs, health literacy, and program specifications.
• Comply with program-specific contractual agreements, company policies, quality standards, and compliance obligations.
• Engage in program-specific customer meetings, training sessions, and orientations.
• Exhibit clinical competency and system proficiency through mandatory assessments.
• Assist with special projects and additional responsibilities aligned with contract performance and business requirements.
• Active, unrestricted Registered Nurse (RN) license in the primary state of residence.
• Compact license is required.
• A minimum of two (2) years of RN clinical experience.
• Proficiency in Microsoft Office applications.
• Basic proficiency in database navigation.
• Excellent verbal and written communication skills.
• Capability to manage multiple priorities and adhere to deadlines.
• Ability to work independently in a remote setting.
• Internet connection via cable broadband or fiber optic service with a minimum of 10Mbps download/5Mbps upload speed when working from home.
• Bachelor of Science in Nursing (BSN) or active enrollment in a BSN program is preferred.
• Preferred experience in case management or care coordination.
• Familiarity with pharmacy operations, pharmaceuticals, life sciences, or healthcare business environments is preferred.
