RN Case Manager – Transition of Care, Bilingual Spanish or Vietnamese

This is a fully remote position, open to applicants in California.

📋 Description

• Assist members as they transition from hospital or skilled nursing facilities

• Aim to decrease readmission rates

• Organize post-discharge care and services among providers and community resources

• Create and oversee individualized discharge care plans

• Inform members and their families about diagnoses, medications, and self-care practices

• Track progress and ensure seamless continuity of care

• Champion member needs and facilitate their recovery

• Deliver care management services via telephone

• Engage, educate, collaborate, and foster positive relationships with members, management, staff, and vendors

• Evaluate mathematical computations, basic statistics, and statistical reports

• Effectively prioritize various tasks and manage suitable resolutions

⛳️ Requirements

• Valid, unrestricted California RN license

• Bilingual in Spanish or Vietnamese

• Minimum of 2 years of case management or clinical experience

• Must be open to obtaining licensure in other designated states within the first 6 months of employment: Nevada (non-compact), Arizona, North Carolina, and Texas (compact)

• Excellent communication and care coordination abilities

• Familiarity with Medicare Managed Care Plans, insurance regulations, and community resources

• Capacity to prioritize multiple tasks and engage in advanced problem-solving

• Strong organizational and time management skills

• Ability to understand and analyze statistical reports

• Must be available to work Monday–Friday, 8am–5pm Pacific Time

• Preferred: Certification in Case Management
