
RN Case Manager, Home Hospice – Fulltime, Day

This is a fully remote position, open to applicants in United States.
• Deliver clinical home hospice services while traveling daily to various hospice patients within the Wellstar region.
• Take primary responsibility for patients with terminal illnesses and their caregivers by employing the nursing process.
• Collaborate with physicians, the Hospice Medical Director, and the interdisciplinary team to coordinate care.
• Assess and organize services from clinical care partners, social workers, chaplains, bereavement coordinators, volunteers, pharmacies, DME providers, and contracted staff.
• Maintain ongoing communication with patients and families through phone calls and personal visits.
• Provide education to patients and caregivers regarding disease processes, symptom management, medications, nutrition, and indicators of impending death.
• Oversee medication management and evaluate symptoms, medication effectiveness, disease progression, safety, caregiving adequacy, potential abuse or neglect, and suicide risk.
• Revise individualized care plans and facilitate transitions in care level or transfers to hospice facilities.
• Perform physical assessments, consult with physicians, and pronounce patients at the time of death.
• Engage in interdisciplinary meetings, family conferences, case studies, performance improvement initiatives, committees, hiring processes, orientation, education, peer review, and case study analysis.
• Design and supervise personal care plans for hospice aides.
• Maintain electronic medical records and the medical elements of care plans.
• Monitor the quality of nursing care, safety, infection control, customer satisfaction, and complaint resolution.
• Act as a clinical resource for PRN RNs, on-call RNs, LPNs, and CCPs.
• Adhere to Wellstar policies, work standards, code of conduct, state and federal licensure requirements, and JCAHO standards.
• Carry out additional responsibilities as assigned.
• An Associate's degree in Nursing or a Nursing Diploma is required; a Bachelor's degree in Nursing is preferred.
• A current RN license: either single-state or RN compact/multistate license.
• BLS certification is mandatory upon hire.
• A Georgia driver's license or a valid state driver's license is required.
• Private vehicle insurance is necessary.
• A minimum of 2 years of acute care or hospice/home care experience is preferred.
• Hospice RN experience is strongly preferred.
• Outstanding empathy and customer service skills are essential.
• Strong assessment and clinical judgment abilities are required.
• Familiarity with developmental characteristics and disease processes affecting individuals of all ages is important.
• Proficient written and verbal communication skills are needed.
• Effective use of computers for patient care documentation is required.
• Personal vehicle is necessary.
• CHPN certification is preferred upon hire.
