RN Case Manager, Experienced

This is a fully remote position, open to applicants in Washington.

📋 Description

• Facilitate and coordinate effective, high-quality care for patients with ESRD and CKD.

• Reduce fragmentation within the healthcare delivery system.

• Collaborate on care coordination among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.

• Operate both independently and in collaboration with healthcare team members.

• Evaluate patient needs and create, as well as oversee, personalized care plans.

• Manage services across interdisciplinary teams.

• Assist patients during complex transitions in healthcare.

• Fulfill business and patient requirements through travel within the designated territory.

• Participate in nephrology partner meetings on a monthly or quarterly basis as necessary.

• Modify territory assignments based on business demands.

⛳️ Requirements

• An A.D.N. degree from an accredited nursing school is required; a B.S.N. is preferred; a three-year diploma from an accredited program may substitute for a nursing degree.

• A current Registered Nurse (RN) license in the state of practice is required.

• Maintaining continuing education credits as mandated by the state of practice is required.

• A minimum of five (5) years of clinical nursing experience is required.

• At least three (3) years of renal nursing experience is preferred.

• Case management experience is highly preferred, encompassing the assessment of patient needs, development and management of individualized care plans, coordination of interdisciplinary services, and support during complex healthcare transitions.

• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.

• Familiarity with data management regarding clinical, financial, and patient satisfaction outcomes.

• Proven experience and effectiveness in a change agent role.

• Understanding of Continuous Quality Improvement (CQI) techniques.

• Prior experience in healthcare performance coaching is required.

• Current CPR certification is required.

• Ability to adapt to new electronic processes and increased productivity expectations for Capella implementation.

• A valid driver’s license in the state of employment, a positive driving record, and the capacity to meet insurance coverage requirements are required.

• Basic computer skills and proficiency in MS Word and Outlook are required.

• Functional proficiency with DaVita clinical software programs, including Capella, is mandatory within 90 days of employment.

• A home office with internet connectivity of at least 1MB upload and 1MB download speed is required.
