This is a fully remote position, open to applicants in California, +3 more states.
• Coordinate and facilitate high-quality, cost-effective care for ESRD and CKD patients.
• Reduce fragmentation in the healthcare delivery system.
• Operate autonomously while collaborating with the healthcare team.
• Coordinate care among patients, their families, physicians, providers, care teams, communities, and the Regional Operations Manager.
• Provide support to ESKD and CKD patients across California, Oregon, Washington, and Nevada as necessary.
• Adjust territory assignments according to business requirements.
• Offer remote float case management coverage from Monday to Friday.
• Address patient needs and accommodate nephrology partner availability.
• A.D.N degree from an accredited nursing school is required; B.S.N is preferred; a three-year diploma from an accredited program may substitute for a nursing degree.
• A current Registered Nurse (RN) license in the state of practice is required, with licensure in California, Oregon, Nevada, and Washington necessary, or the willingness to obtain any required state license.
• Continuing education credits must be maintained as mandated by the state of practice.
• Experience in case management is required, or at least three years of experience in assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions.
• A minimum of five years of clinical nursing experience is strongly preferred.
• At least three years of renal nursing experience is preferred.
• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
• Knowledge of data management and clinical, financial, and patient satisfaction outcomes is necessary.
• Experience and effectiveness in a change agent role are required.
• Familiarity with Continuous Quality Improvement (CQI) techniques is essential.
• Previous experience in healthcare performance coaching is required.
• A current CPR certification is mandatory.
• Ability to adapt to new electronic processes and meet increased productivity expectations for Capella implementation is necessary.
• A valid driver’s license in the state of employment, a positive driving record, and the ability to meet insurance coverage requirements are required.
• Basic computer skills and proficiency in MS Word and Outlook are required.
• Functional proficiency with DaVita clinical software programs, including Capella, must be achieved within 90 days of employment.
• A home office with internet connectivity that provides at least 1MB upload and 1MB download speed is necessary.
