RN Case Manager

This is a fully remote position, open to applicants in Connecticut.

📋 Description

• Establish meaningful connections with patients and their families.

• Comprehend patients' aspirations, challenges, and driving factors.

• Develop tailored care plans that address medical, behavioral, social, and emotional requirements.

• Coordinate care among different providers and settings.

• Ensure the safety, quality, and continuity of healthcare services.

• Educate and empower members to make well-informed decisions regarding their healthcare.

• Assist members in navigating the healthcare landscape.

• Collaborate with a multidisciplinary team.

• Share valuable insights and advocate for comprehensive health.

• Utilize data to pinpoint opportunities and assess impact while keeping the patient at the forefront.

• Uphold compliance with regulatory standards.

• Engage in team activities, training, and development with minimal travel required.

⛳️ Requirements

• A minimum of 5 years of experience as a Registered Nurse.

• Active RN license with compact, multi-state licensure.

• Strong expertise in care management.

• Experience in managing complex and chronic conditions.

• Proficient in motivational interviewing techniques.

• Skilled in patient education and advocacy.

• Capability to work independently in a remote setting while maintaining team connectivity.

• Comfortable with technology and various EMR systems.

• Willingness to travel 5% to 10% for team activities, training, and development.

• Employees must provide their own internet connection.

• Required internet speed of at least 100 Mbps for download and 10 Mbps for upload.

• A Bachelor's Degree in Registered Nursing is preferred.

• Specialty Certification in Care Management (CCM) is preferred.

• Multilingual abilities are a plus.

• Compliance with security policies and procedures to protect PHI, PII, and company intellectual property is mandatory.

• Disclosure of any secondary employment is required.

• Must be available during specified working hours.

• Participation in virtual meetings with cameras on is required.
