RN Care Manager – OB

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Design, evaluate, and implement intricate care management strategies for members primarily requiring physical assistance.

• Assess member needs, obstacles to accessing care, and social determinants impacting health.

• Create continuous care plans and work collaboratively with providers, specialists, and community organizations.

• Identify challenges and barriers to care, offering effective care management interventions.

• Act as a liaison among members, caregivers, and care provider teams to guarantee suitable person-centered care.

• Monitor and follow up on member status, changes in condition, and progress towards care plan objectives.

• Offer resource support for services in areas such as employment, housing, independent living, justice, and foster care.

• Ensure timely access to necessary care and services.

• Conduct outreach through telephonic, digital, home visits, and/or other locations.

• Collect, record, and preserve member data and care management activities to ensure regulatory compliance.

• Educate members and their families/caregivers about disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.

• Provide leadership insights on ways to enhance care quality and improve cost-effectiveness.

• Perform additional assigned tasks while adhering to company policies and standards.

⛳️ Requirements

• Degree from an accredited nursing school or a Bachelor's degree in Nursing.

• 2–4 years of relevant experience.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.

• Ability to work remotely from anywhere within the state of Missouri.

• Monday–Friday, 8 am–5 pm work schedule.
