This is a fully remote position, open to applicants in Wisconsin.
β’ Evaluate and assess the needs and requirements of members to attain or sustain optimal wellness.
β’ Direct members and their families to suitable care and wellness resources.
β’ Facilitate connections with community and healthcare resources.
β’ Address members' physical, environmental, and psychosocial health challenges using diverse strategies and techniques.
β’ Identify and eliminate obstacles to effective care delivery.
β’ Oversee patient care through continuous assessments and evaluations.
β’ Ensure that patients progress towards their desired health outcomes.
β’ Develop member care plans as necessary.
β’ Interpret guidelines and autonomously determine the most appropriate courses of action.
β’ Make decisions regarding work methods in situations of uncertainty with minimal supervision.
β’ Adhere to established guidelines and procedures.
β’ Must be a Registered Nurse with at least 2 years of experience in in-home case/care management.
β’ Experience working with adult populations is essential.
β’ Familiarity with community health and social service agencies, as well as additional community resources.
β’ Willingness to travel to members' homes within a 30 to 40-mile radius.
β’ Outstanding communication and interpersonal skills, with the ability to quickly establish rapport.
β’ Ability to work independently within the role and scope of responsibilities.
β’ Proficiency in using electronic information applications/software programs, including electronic medical records.
β’ Intermediate to advanced computer skills, including experience with Microsoft Word, Outlook, and Excel.
β’ Strong keyboard and web navigation abilities.
β’ Willingness to work a full-time, flexible schedule of at least 40 hours per week.
β’ Possession of a valid state driver's license.
β’ Personal vehicle liability insurance that meets state minimums or specified higher limits.
β’ Valid driver's license, car insurance, and access to a reliable automobile.
β’ A separate room with a locked door available for use as a home office to ensure continuous privacy.
β’ Access to high-speed DSL or cable modem; satellite internet service is not permitted.
β’ Required TB screening for this patient-facing position.
β’ A strong passion for enhancing consumer experiences.
β’ BSN is preferred.
β’ 3β5 years of in-home assessment and care coordination experience is preferred.
β’ Experience in health promotion, coaching, and wellness is preferred.
β’ Previous experience in managed care is preferred.
β’ Bilingual proficiency in English and Spanish is preferred.
β’ Certification in Case Management is preferred.
β’ Knowledge of and/or certification in Motivational Interviewing is preferred.
