This is a fully remote position, open to applicants in Michigan.
β’ Oversee a caseload of high-risk patients and address their care challenges.
β’ Execute Transitional Care Management, Chronic Care Management, Disease Management Education, and Medication Education.
β’ Create and oversee patient care plans.
β’ Act as co-chair of the pod in collaboration with the pod leader, emphasizing prioritization of patient needs and enhancement of outcomes.
β’ Coordinate care services to deliver comprehensive support throughout the healthcare journeys of patients.
β’ Work closely with the care team to formulate personalized care plans, tackle care gaps, and access necessary resources.
β’ Manage transitions of care to avert preventable hospital admissions.
β’ Organize and facilitate High Risk Huddles, ensuring completion of follow-up actions.
β’ Rank patients based on the severity of their condition and urgency.
β’ Analyze medical records to pinpoint care gaps and arrange solutions.
β’ Perform telephone nursing assessments.
β’ Provide education to patients and caregivers regarding diseases, medications, health maintenance, and prevention strategies.
β’ Record interactions, assessments, and updates in medical records.
β’ Serve as a liaison among patients, providers, and available resources.
β’ Ensure clear communication of patient status and care plans during transitions between home, hospital, post-acute care, and back home.
β’ Current Registered Nurse License.
β’ Minimum of 2 years of experience in care management within community, health plan, or hospital systems.
β’ Strong clinical skills and proactive problem-solving abilities.
β’ Excellent communication skills.
β’ Capability to conduct thorough telephone assessments.
β’ Familiarity with Medicare regulations and standards for home care and hospice.
β’ Experience in small group presentations and teaching/training.
β’ Demonstrates outstanding interpersonal skills.
β’ Possesses strong written and verbal communication skills.
β’ Proficient in computer applications (email, Word, Excel, PowerPoint, etc.).
β’ Effectively manages time to ensure all responsibilities and documentation are completed promptly.
β’ May be required to obtain multi-state licensing.
