RN Care Manager

This is a fully remote position, open to applicants in California, +1 more state.

📋 Description

• Oversee a caseload of patients with intricate or chronic health issues

• Perform thorough clinical evaluations and create personalized care plans

• Direct care coordination among multidisciplinary teams and community resources

• Supervise hospital discharge planning, post-acute placements, and follow-up monitoring

• Modify care plans in response to evolving patient conditions and escalate when necessary

• Collaborate with payers regarding utilization management, authorization protocols, and documentation

• Keep case documentation up to date and assist in quality monitoring and reporting

• Promote value-based care and health equity initiatives

⛳️ Requirements

• Valid RN-level clinical credentials and capability to practice within the RN scope of practice

• Experience or familiarity in clinical case management, care coordination, utilization management, or similar clinical roles

• Proficiency in assessing complex medical, behavioral, and social requirements

• Skill in creating and executing personalized care plans

• Capability to coordinate care among physicians, specialists, social workers, payers, and community resources

• Understanding of hospital discharge processes, post-acute placements, and follow-up care

• Ability to utilize clinical judgment within established protocols and escalate as needed

• Competence in maintaining precise and timely case documentation

• Capacity to engage in quality monitoring and reporting efforts
