RN Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate the needs for transitional care

• Coordinate services throughout the care continuum

• Facilitate the progression of care

• Assist patients in safely transitioning from the hospital

• Collaborate with physicians, utilization review nurses, interdisciplinary care teams, patients, and their families

• Address care requirements and eliminate obstacles to discharge

• Participate in interviews with hiring leaders for RN Care Coordinator roles during the virtual hiring event

⛳️ Requirements

• Valid Registered Nurse (RN) license

• Minimum of one year of nursing experience in an acute care environment

• Current Basic Life Support (BLS) certification

• Must be authorized to work in the United States
