Relief Referral Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Receive, assess, and process incoming referrals and associated documentation from external providers using Epic and various referral sources.

• Validate the completeness of referrals, including demographics, referring provider information, diagnoses/procedures, requested services, and supporting documentation.

• Interpret medical terminology, diagnostic language, urgency indicators, and procedural requests accurately.

• Review and input diagnoses using ICD10 codes or approved free-text selections.

• Utilize intake, diagnosis, and specialty-specific referral matrices to establish appropriate routing.

• Evaluate and assign routine, urgent, or emergent referral priority levels.

• Create and maintain referral records in Epic, detailing referral type, department, location, and point of service.

• Attach relevant documentation to the appropriate Epic referral and verify patient identity.

• Identify incomplete, misdirected, or inappropriate referrals and redirect them for resolution.

• Communicate with referring providers, clinic staff, internal stakeholders, and occasionally patients to gather missing information.

• Clarify referral intent, urgency, or destination when documentation lacks clarity or presents conflicts.

• Document intake actions, decisions, and discrepancies within Epic.

• Escalate complex referrals, uncertainties in routing, access barriers, and safety concerns.

• Support centralized access, outpatient clinics, and diagnostic services from referral receipt to routing for scheduling and clinical review.

• Identify workflow barriers and quality issues affecting referral accuracy, timeliness, and patient access.

• Report challenges, issues, and potential risks to leadership.

• Participate in enhancing intake workflows, referral accuracy, and Epic system performance.

• Review and suggest updates to referral matrices.

• Validate matrix accuracy in collaboration with leadership.

• Perform additional tasks supporting Centralized Access Service operations.


⛳️ Requirements

• Bachelor’s degree with 2 years of experience in an ambulatory clinic, OR Associate’s degree with 4 years of experience in an ambulatory clinic, OR High School diploma with 6 years of experience in an ambulatory clinic.

• Familiarity with various ambulatory workflows.

• Completion of a medical terminology course is preferred.

• Prior experience in a centralized access center or medical referral management role is preferred.

• Experience in supporting patient scheduling workflows is preferred.

• Epic electronic medical record experience is preferred.

• Experience in performing detailed, high-volume work requiring precision and independent judgment.

• Background in reviewing, interpreting, and processing medical documents.

• Experience in managing or processing referrals across multiple clinics or services.

• Working knowledge of medical terminology, diagnoses, procedures, and diagnostic testing.

• Ability to interpret ICD10 diagnosis codes and clinical referral documentation.

• Proficiency in Epic referral workflows, including referral creation, appointment requests, diagnosis entry, documentation, and attachment management.

• Understanding of ambulatory specialty clinic structures and diagnostic referral workflows.

• Knowledge of patient identification standards, confidentiality requirements, and HIPAA regulations.

• Ability to prioritize competing tasks in a high-volume environment while ensuring accuracy and compliance.

• Strong verbal and written communication skills.

• Commitment to patient-centered access, service excellence, and equitable care delivery.

• Capacity to maintain focus and attention to detail throughout the workday.

• Prolonged sitting and sustained computer use; extensive keyboard and mouse operation.
