Registered Nurse – Virtual Care, Care Management

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee medically intricate patients across various care programs, such as CCM, RPM, RTM, and APCM.

• Prepare detailed Annual Wellness Visit documentation to enhance provider efficiency and ensure CMS compliance.

• Conduct virtual patient consultations via Zoom, reviewing medical, surgical, and family histories, medications, screenings, immunizations, and assessments.

• Identify overdue screenings, risk factors, and gaps in care.

• Relay patient summaries and critical findings to healthcare providers.

• Collaborate with patients to gather missing or clarified information.

• Provide care management through phone calls and online portals.

• Execute thorough nursing assessments.

• Create, implement, monitor, and revise personalized care plans.

• Utilize motivational interviewing techniques to engage patients and their caregivers.

• Track progress, reassess objectives, pinpoint barriers, and modify care plans accordingly.

• Update primary care providers and interdisciplinary teams on patient status and developments.

• Deliver patient education, coaching, and support.

• Serve as a patient advocate and escalate issues when necessary.

• Identify and address preventive, laboratory, medication, and follow-up care gaps.

• Coordinate orders, referrals, follow-up actions, services, and providers.

• Maintain precise, timely, and confidential patient records.

• Utilize CT360, RXNT, Zoom, EHR systems, and telehealth platforms.

• Support clinical quality, patient engagement, and satisfaction outcomes.

• Adhere to HIPAA, CMS, and organizational compliance standards.

• Engage in professional development and ongoing training.

• Perform additional duties as assigned.

⛳️ Requirements

• Valid, unrestricted Registered Nurse (RN) license in a compact state.

• Willingness and capability to obtain additional state licenses upon employment (company funded).

• At least 3 years of clinical nursing experience, including bedside, Primary Care, Med/Surg, or Home Health experience.

• Experience delivering care in a telehealth or virtual care environment.

• Experience working remotely.

• Background in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), telehealth or virtual care coordination, primary care, or care management.

• Proficiency with EHR systems and telehealth platforms.

• Strong skills in clinical assessment and care coordination.

• Exceptional verbal and written communication abilities.

• High level of organization, attention to detail, and time management skills.

• Ability to operate independently in a remote setting.

• Commitment to patient-centered and culturally competent care.

• Dedicated, private workspace at home.

• Reliable internet connection.

• Compatible computer: Windows or Apple (macOS) only; no Chromebooks, Linux machines, or smartphones.

• Minimum operating systems: Windows 10 or macOS 14 Sonoma.

• Required specifications: Intel Core i5 or AMD equivalent processor for Windows, or Apple Silicon for Macs; 16 GB RAM.

• Wired headphones are necessary; built-in microphones are not permitted.

• Minimum internet speed: 100 Mbps download and 50 Mbps upload.

• Ethernet cable must connect the computer directly to the router.

• Must use Google Chrome with Amazon Workspaces.

• Webcam required for video conferencing.

• Preferred qualifications: Experience with Medicare Annual Wellness Visits, knowledge of CMS documentation and preventive care, certification in case management or gerontological nursing, and experience with various virtual care programs and platforms.
