Registered Nurse, Virtual

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Deliver remote clinical supervision, telephonic triage, and care coordination.

• Execute virtual clinical intakes, health screenings, and wellness assessments with patients and their caregivers.

• Conduct telephonic triage and clinical evaluations for incoming patient calls, directing care in accordance with established protocols.

• Review and revise care plans in accordance with physician directives and state regulations.

• Monitor Remote Patient Monitoring (RPM) data and vital trends, identifying clinical changes for the care team.

• Act as the primary virtual contact for patients, families, attending physicians, and home care personnel.

• Ensure accurate and timely clinical documentation within EHR platforms.

• Engage in virtual case discussions, team meetings, and quality assurance evaluations.

• Collaborate with physicians and interdisciplinary teams to guarantee high-quality care and adherence to regulatory standards.

⛳️ Requirements

• A current, unencumbered Registered Nurse (RN) license in the state of New Jersey is required.

• Multi-state Compact Licensure is preferred.

• A minimum of 1–2 years of clinical experience in home health, triage, or remote care management is necessary.

• Strong skills in clinical triage, assessment, and critical thinking.

• Professional and empathetic communication skills across phone and video platforms.

• Proficient in Google Workspace and EHR care management systems.

• A dedicated home office setup with high-speed internet that complies with all HIPAA privacy requirements.
